I work with Lactation Network to provide 2-3 consultations (In-home, In-office, or Virtual) at no-cost to the you. Client must check for pre-approval through Lactation Network prior to scheduling appointment as not all plans will provide coverage. While filling out the pre-approval form on their site, click "Yes" after "Did a Lactation Consultant Send You" and "Elizabeth Ferrall" under name of consultant. This ensures that I get notice of insurance coverage. You will receive notice via email within the day to let you know if you are eligible for coverage by your insurance.
My prices can be seen under Lactation Appointments or Nutrition Services on this site. Lactation and Nutrition services qualify as an HSA or FSA expense. I collect payment at the time of service, then I will provide you with a superbill containing all applicable billing codes that your insurance will need to provide coverage. Under the Affordable Care Act (ACA), most insurance plans are required to cover lactation services at no out-of-pocket costs. Getting them to actually reimburse payment can sometimes be difficult though. It is recommended that you speak to your insurance rep prior to lactation services to get pre-approval, which can sometimes help the reimbursement process. Many families report having to submit a superbill several times before receiving reimbursement. Here is an article by Rachel O'Brien that may be able to help aide you in this process. Also check out this article by the National Women's Law Center for more information on steps for reimbursement.
Nutrition services are hit-or-miss with coverage, but the same recommendations listed here do apply. I will provide a superbill following nutrition consults with applicable codes as well. Even though Rachel O'Brien's above article is directed towards breastfeeding coverage, the same steps can be taken for reimbursement of nutrition consulting.
